# Diamox for the Everest Base Camp Trek: Clinician Questions, Acclimatization and Safety

A clinician should decide whether acetazolamide is appropriate for your Everest Base Camp itinerary after reviewing your health, medicines, allergies, pregnancy or kidney and liver concerns. Diamox can support acclimatization for some travellers, but it does not replace a staged ascent, symptom monitoring or descent.

## Stop, descend and seek help when symptoms worsen

Online guidance cannot diagnose illness. Tell your guide early and seek professional medical advice whenever symptoms are concerning.

**1 · Monitor**Mild headache or appetite change: rest, hydrate and report it.

**2 · Do not ascend**Persistent headache, nausea, dizziness or unusual fatigue means no higher sleeping altitude.

**3 · Descend / evacuate**Confusion, loss of coordination, breathlessness at rest or a cough with frothy sputum are emergencies.

- **Stop**Pause the ascent and assess with your guide.

- **Descend**Go lower if symptoms do not settle or are severe.

- **Evacuate**Use medical care, rescue coordination and insurance support when needed.

## Start with the itinerary and your health

Take the exact route, sleeping-altitude table, highest point and walking days to a qualified clinician. Discuss previous altitude illness, kidney or liver disease, pregnancy, allergies, regular medicines and the symptoms that should stop the trek. An online dose or a friend's tablet cannot answer those questions safely.

## Medication works with the itinerary

- Keep acclimatization nights in Namche, Dingboche or the route plan advised by the operator and clinician.

- Ascend gradually and do not increase sleeping altitude while symptoms are worsening.

- Hydrate normally, eat what you tolerate and avoid using medication to justify a rushed stage.

- Keep the medicine in original packaging and carry the written instructions.

- Tell the guide what you take and where the emergency plan is stored.

## Know what Diamox cannot do

Acetazolamide does not make an unsafe ascent safe, prevent every form of altitude illness or replace a descent decision. Confusion, poor coordination, breathlessness at rest, a wet cough or rapidly worsening symptoms need urgent professional action regardless of medication.

## Build a stop and descent plan

Before the high stages, agree who checks symptoms, who calls the operator or insurer, where the nearest realistic descent is and how a traveller reunites with the group. Current weather, flights, roads, clinics and evacuation capacity can change, so reconfirm the plan locally.

## Clinician and operator checklist

- Bring the exact itinerary and sleeping-altitude table to the appointment.

- List every medicine, supplement, allergy and relevant medical condition.

- Record the prescribed instructions and side effects in a travel note.

- Confirm guide, insurer, clinic, evacuation and early-return contacts offline.

- Treat worsening symptoms as a route decision, not a medication problem to hide.
